How do I schedule an office visit or procedure?
Our Office Assistants and Schedulers can schedule office visits and procedures.  Please call to schedule your next appointment, 309-454-5900.

What should I bring to my appointment?
Please bring your insurance card, photo ID and referral if needed. Payment is expected at the time of service.

How do I get my test results?
Please call to schedule a follow up appointment so your physician can go over this information with you.

What do I do when I need a refill on my medication?
Please call your pharmacy to have them send in a refill request to us.

Where is your office located/Where should I park?
Our Main Office is located across the drive from Advocate BroMenn Medical Center, in the Medical Office Center 1.  The address is 1302 Franklin Avenue, Suite 4800 in Normal.  You can park in the MedPark 6 Parking located north of our building or at any of the parking lots on the Advocate BroMenn Medical Center campus. We have valet parking available in front of our building as well.

Advocate Eureka Hospital is located at 101 S. Major Street in Eureka. Parking is conveniently located in front of the building.

Dr. John Warner Hospital is located at 422 W. White Street in Clinton. Parking is conveniently located in front of the building.
